Restore Rehab is an out of network clinic. This means we do not bill your insurance. If requested, you will be provided with a superbill that you can then submit to your insurance for any available out-of-network benefits. You will need to call your insurance company and inquire if you have out of network Occupational Therapy benefits. Here are a few helpful questions to ask when calling your insurance company.

  • What are my Occupational Therapy benefits?

  • What is my out of network percentage of coverage?

  • Do I have a deductible?

  • If yes, how much is it?

  • How much of the deductible has been met?

  • Do I need a written referral from a doctor? (please note Restore Rehab requires a referral if you have a medical condition per state law-see above)

  • Does the referral need to come from my primary care doctor?


As an Occupational Therapist in Washington, Karen is able to evaluate people before obtaining a medical prescription or referral.  According to the state law, we can work together if we determine that you are dealing with something that is “not a medical case” (you have an absence of pathology, or the pathology has stabilized) and we are treating your functional deficits.
